Case Name: Create-a-Form

Summary: A operator of this
tool is prompted to enter queries, and all possible responses (up to 5) to each
query. The number of queries is not
limited. These query/responses will be constructed
into a feedback form.
Basic
Course of Events:

The operator is prompted for a form name.
2.
The operator is prompted for query and response sets
3.
When operator indicates DONE, acknowledgement is
provided
